
The Senior Extended Services Project was created to bridge the gap between healthcare services and everyday living needs. While many seniors qualify for Medicaid, Medicare, or other aging programs, essential support services often remain uncovered. These gaps can lead to preventable hospitalizations, caregiver burnout, housing instability, and loss of independence. Built on the 26-year foundation of Able Home Health Care, our nonprofit program extends support beyond clinical care to ensure that seniors and adults with disabilities can remain safely and comfortably at home.
